A parent or legal guardian signs this form for each child attending a birthday party. One form per child.

How it works

  1. Fill out the form and sign at the bottom.

Shared with the party Host, who provides all food. Please also tell the Host directly.

Agreement and release
  1. Card ripping. My child may open sealed trading card packs. Pack contents are random, and cards are handled as the party Host has arranged. Trades between children are between those families; Tombstone doesn't approve or reverse them.
  2. Food. All food and drinks are provided by the party Host, not Tombstone. I have told the Host about any allergies.
  3. Supervision. The party Host and attending adults supervise the children. Tombstone staff do not babysit.
  4. No medical care. Tombstone employees are not trained in first aid or CPR. In an emergency, staff will call 911.
  5. Property. Tombstone isn't responsible for lost or damaged personal items or cards.
  6. Release. I release Tombstone Collectibles, LLC, its owners, employees, and agents from claims for injury or property damage arising from the inherent risks of attending the party, except claims caused by gross negligence or intentional misconduct. This form is governed by Florida law.
NOTICE TO THE MINOR CHILD'S NATURAL GUARDIAN (Fla. Stat. § 744.301(3)) READ THIS FORM COMPLETELY AND CAREFULLY. YOU ARE AGREEING TO LET YOUR MINOR CHILD ENGAGE IN A POTENTIALLY DANGEROUS ACTIVITY. YOU ARE AGREEING THAT, EVEN IF TOMBSTONE COLLECTIBLES, LLC USES REASONABLE CARE IN PROVIDING THIS ACTIVITY, THERE IS A CHANCE YOUR CHILD MAY BE SERIOUSLY INJURED OR KILLED BY PARTICIPATING IN THIS ACTIVITY BECAUSE THERE ARE CERTAIN DANGERS INHERENT IN THE ACTIVITY WHICH CANNOT BE AVOIDED OR ELIMINATED. BY SIGNING THIS FORM YOU ARE GIVING UP YOUR CHILD'S RIGHT AND YOUR RIGHT TO RECOVER FROM TOMBSTONE COLLECTIBLES, LLC IN A LAWSUIT FOR ANY PERSONAL INJURY, INCLUDING DEATH, TO YOUR CHILD OR ANY PROPERTY DAMAGE THAT RESULTS FROM THE RISKS THAT ARE A NATURAL PART OF THE ACTIVITY. YOU HAVE THE RIGHT TO REFUSE TO SIGN THIS FORM, AND TOMBSTONE COLLECTIBLES, LLC HAS THE RIGHT TO REFUSE TO LET YOUR CHILD PARTICIPATE IF YOU DO NOT SIGN THIS FORM.
